HOLD HARMLESS and TRAINING AGREEMENT
By submitting this electronic enrollment form, you agree that this is a legally binding contract that
becomes effective immediately.  It cannot be postponed, cancelled, transferred or refunded unless by
written agreement of both parties.  Enrollee
/handler (this would be you) agrees and promises to pay the
total price agreed upon, and understands The Pet Teacher, Denise Lukacs or instructor will not refund any
amount unless by written agreement of both Enrollee and The Pet Teacher.  Class reservations are made on
a first-paid, first-booked basis.  If The Pet Teacher has not received payment within 24 hours of booking
the session date(s), all session dates being held will be cancelled without further notice.  

Since there are many variables in the owner/handler environment & practice schedule/commitment that are
beyond The Pet Teacher's control, The Pet Teacher, Denise Lukacs or instructor cannot accept
responsibility for the progress of your dog's training.  Additionally, if The Pet Teacher instructor believes
that the handler is not making the anticipated progress due to intentional disregard of instruction, the
instructor may immediately cancel sessions without notice and no refund.

The dog's behavior now and in the future is solely the responsibility of the owner/handler of the dog.  The
Pet Teacher, Denise Lukacs or instructor will not be liable for any damage or loss resulting from failure of
the dog to respond to any cues or behaviors taught by The Pet Teacher  or resulting from advice supplied to
the enrollee/handler of the dog from other sources.  Should any behavior on the dog's part now or in the
future result in damage to property, owners or person or property of some third party, the dog's owner or
handler agrees to assume full responsibility for any and all such damage and to absolve The Pet Teacher,
Denise Lukacs and all employees and agents from any and all obligations to pay such damage to owner,
handler, or third party.  

If the handler cannot make a session, a four-hour notice must be provided to The Pet Teacher instructor or
the handler will forfeit the session with no refund or rescheduling.  Extreme circumstances will be
considered on an individual basis.  If we arrive and your dog is sick s
o we cannot train,, it still counts as a
session.   Please plan on devoting your attention to the session because additional time
incurred will be
charged accordingly
if you take phone calls, answer the door, meet with the landscapers, do homework for
your children, or cook your meals while we are in session.
 Please do not eat while we are working because it
is disruptive to your dog's focus.

If you decide not to keep your dog due to your personal reasons, such as the dog is more work than
anticipated, or your schedule doesn't permit you enough time to take care of your dog - that is not a
training issue and there are no refunds for unused sessions.  It is our hope that you will keep your dog and
understand that dogs are not disposable.  

If weather (snow, ice, thunderstorms, fog, hail, heat or cold making it unsafe for the dog or for the
instructor to be traveling on the road) becomes an issue and the training cannot be safely or productively
conducted within your home, the instructor will contact the handler no later than one hour prior to the
session start time.  Weather can change dramatically within a short period of time, so the instructor will
make the decision based on experience as soon as deemed appropriate.  Even if you think it's safe for your
dog to work in a weather condition, The Pet Teacher instructor will make the final decision based on our
experience.  Sessions having to be postponed due to the weather or instructor illness will be automatically
rescheduled and are the only sessions exempt from the 45-day window.

Children should never be left unsupervised with a dog.  An adult must accompany your child at all times and
be responsible for the dog's learning, even if the child is the primary handler.  

The use of choke chains, prong collars, shock collars, spray water bottles, training chain bags, scruff
shakes, rollover corrections, water bottles and hitting is strictly prohibited during the weeks of our class
sessions.  If the handler
(you) uses these items/techniques during the duration of the training sessions it
can negatively affect the learning results with the dog.  Therefore, use of these items/techniques during
the training class window will be immediate grounds for termination of all sessions with no refund.

The Hold Harmless and Training Agreement shall remain in effect for the duration of the elected class
session, as well as be applicable for any future classes with The Pet Teacher.

ENROLLMENT PROCESS
After phoning me to discuss dates, next please complete the electronic form below along with completing
your appropriate deposit or payment.   Class sessions are conducted in your home and surrounding locations
as deemed appropriate by The Pet Teacher instructor.  All
paid sessions must be completed within 45 days
unless other arrangements have been agreed upon.  Class materials you must provide at the sessions include
a 6' leather or nylon leash, leather or nylon flat collar, and soft training treats.  
The Pet Teacher Training Promise
We guarantee to our clients that if the handler we work with follows our training instructions
along with our practice requirements, and the issue with your dog persists, we will provide
assistance for up to three months from start of training at no-charge on those exercises we
have practiced.  Assistance may be provided in-home or via phone and/or email support, as
determined most appropriate by The Pet Teacher instructor.
